Hold Right + A + B on controller two and press Start on controller one at the title screen. The "No damage" option should now be unlocked.

Extra items

Place a fish in a bubble, then throw it at one of the "notches" at the upper or lower part of the screen to collect a large heart, small heart, fork, or pipe.
